*** For the 2025-2026 school year FISD will have a new medication form. It will be one form for both over the counter and prescription medications. It will need to be signed by both parent and physician prior to any medications being given on campus.
*** If your student has asthma or an anaphylactic allergy that requires an EpiPen, you will still need to provide the Allergy/Anaphylaxis form along with a new medication form mentioned above for each medication your student will take on campus.
